- Secondary rhinoplasty
Rhinoplasty is probably the most difficult of all cosmetic plastic surgery operations. It is often difficult if not impossible to see the final result at the time of surgery. For this reason some patients may need additional surgery. This occurs in about 5-10% of patients and varys from minor revision to complex restructuring of the nose including grafts.
A great danger is that the surgeon will continue to perform many operations to achieve the desired result and make the patient happy. Unless done carefully this may damage the nose and risks not only a poor result but a disastrous outcome for the patient.
Secondary surgery is not to be undertaken lightly. There are a number of guidelines which have been recommended by surgeons who specialize in secondary rhinoplasty.The surgery should not be performed for at least a year, so that the swelling in the tissue has subsided and the final result seen.
The surgeon and patient must make a clear decision and diagnosis before surgery about exactly what they want to achieve.The ultimate aesthetic goal must be clearly understood. The operation should not just be a "redo" of the original operation. The dissection should be limited to preserve the blood;odd supply and reduce additional scar formation.
If additional material is needed to build up the nose (grafts) it is better to use the patient's own cartilage or bone than to use man made material. Even if all these recommendations are followed a good result is not guaranteed and both surgeon and patient should be cautious about more than one or two revision operations.
