A rhinoplasty can be done either through incisions on the inside of the nose (internal rhinoplasty) or through incisions on the outside of the nose(external rhinoplasty).
The scar for an external rhinoplasty is placed across the columella which is the part of the nose between the nostrils beneath the tip of the nose The incision continues along the inside of the nose. In this way the skin can be lifted away from the cartilage and allow the surgeon to see directly the position and shape of the cartilage.
External rhinoplasty is performed in situations where it is especially necessary to visualize the internal structures of the nose, such as in revision surgery, when repairing septal perforations, cleft lip nose rhinoplasty or in badly misshaped noses.
The advantages are the improved visibility and the precise nature of the surgery, including the positioning of cartilage grafts.
The disadvantages are the visible external scar and the longer time required for surgery.
